Vital Signs Monitoring • Measure and record patient vital signs, including temperature, blood pressure, pulse, and respiration rate. • Report abnormal readings to the nurse or supervising healthcare professional promptly. 2. Hygiene Care • Assist patients with daily personal hygiene tasks such as bathing, grooming, oral care, and dressing. • Maintain patient comfort and dignity throughout all care activities. 3. Diaper Changing & Incontinence Care • Perform timely diaper changes and manage incontinence care. • Ensure proper cleaning, skin protection, and maintenance of patient comfort. 4. Bed Sore (Pressure Ulcer) Prevention & Management • Assist in repositioning patients regularly to prevent pressure ulcers. • Observe and report any signs of skin breakdown to nursing staff. • Help with basic wound care as instructed by the nurse. 5.
